Some scientists believe these responses could translate into a healthier, longer life for humans. So, what does the clinical evidence show? Do these cellular mechanisms really lead to detectable longevity changes in humans? This article examines the research on intermittent fasting and human longevity, as well as the mechanisms that may link the two.


This 2017 review authored by one of the world's leading longevity scientists, Valter Longo, found intermittent fasting improved several aspects of age-related diseases in humans. Various studies included in the review showed that IF protected against type 2 diabetes and cardiovascular disease. The studies included health indicators measured both at baseline and after periods of two to six months of intermittent fasting. Two years later, a second review published in The New England Journal of Medicine (the world's leading medicine journal) showed similar results. The authors concluded that numerous clinical trials indicate that intermittent fasting has broad-spectrum benefits for many health conditions and age-related diseases, including diabetes mellitus, cardiovascular disease, cancers, and neurologic disorders.

In a follow-up study, the researchers found that those factors might contribute to not just a longer but also a healthier life. [2] They saw that women at age 50 who practiced four or five of the healthy habits listed above lived about 34 more years free of diabetes, cardiovascular diseases, and cancer, compared with 24 more disease-free years in women who practiced none of these healthy habits. Men practicing four or five healthy habits at age 50 lived about 31 years free of chronic disease, compared with 24 years among men who practiced none. Men who were current heavy smokers, and men and women with obesity, had the lowest disease-free life expectancy.


A notable aspect of this trial was a very high dropout rate (38%) among the people assigned to the fasting regimen. This may reflect a real-life pitfall of fasting as a weight-loss approach. "It's human nature for people to want to reward themselves after doing very hard work, such as exercise or fasting for a long period of time, so there is a danger of indulging in unhealthy dietary habits on non-fasting days," says Dr. Hu. In addition, there's a strong biological push to overeat following fasting periods. Your appetite hormones and hunger center in your brain go into overdrive when you are deprived of food.


But intermittent fasting may have a beneficial effect on diet psychology for some people, says Kathy McManus, director of the Department of Nutrition at Harvard-affiliated Brigham and Women's Hospital. "One of my patients felt strongly that he didn't want to be bothered with tracking calories and filling out food records. Instead, he opted for a 5:2 fasting approach, which has worked well for him so far." The main goal is to develop a healthy eating pattern that is sustainable and can support weight loss over time, McManus says. As of now, intermittent dieting has not been proved to meet this key criterion.


Part of the fascination with intermittent fasting arises from research with animals showing that fasting may reduce cancer risk and slow aging. "One hypothesis is that fasting can activate cellular mechanisms that help boost immune function and reduce inflammation associated with chronic disease," says Dr. Hu. While it's true that getting rid of excess body fat will improve a person's metabolic profile and lower cardiovascular risk, he says, there's no strong evidence that fasting adds health benefits beyond any other weight-loss strategy.


If you are considering intermittent fasting, make sure to discuss it with your doctor. Skipping meals and severely limiting calories can be dangerous for people with certain conditions, such as diabetes. People who take medications for blood pressure or heart disease also may be more prone to electrolyte abnormalities from fasting. Also, says Dr. Hu, we live in a toxic, obesogenic food environment. So you'll need a strong social support network to endure very low-calorie days over the long haul.

At this time, metabolism shifts from a carbohydrate-burning to a fat-burning state. Some of the fat is used directly as fuel, while some is metabolized into biochemicals called ketones. The state of ketosis brings other changes throughout the body that are thought to underlie some of the health benefits seen with some types of fasting.


On a molecular level, intermittent fasting may extend lifespan in animals because of its effect on genetic DNA. It appears that, in animals, restricting calories may slow down these age-related changes and help them live longer.


Next, decide WHY you want to try intermittent fasting, and write down your health goals. Some reasons may include improved blood sugar levels, manage insulin resistance, weight loss, or healthy aging.


Monitor for potential side effects when starting intermittent fasting. These may include fatigue, weakness, headache, reductions in sexual interest, and a reduced ability to maintain body temperature in cold environments. Many of these side effects are temporary, but if they last longer than a week or two, consider stopping intermittent fasting for a while and see if your symptoms improve.

Dietary restriction (DR) decreases body weight, improves health, and extends lifespan. DR can be achieved by controlling how much and/or when food is provided, as well as by adjusting nutritional composition. Because these factors are often combined during DR, it is unclear which are necessary for beneficial effects. Several drugs have been utilized that target nutrient-sensing gene pathways, many of which change expression throughout the day, suggesting that the timing of drug administration is critical. Here, we discuss how dietary and pharmacological interventions promote a healthy lifespan by influencing energy intake and circadian rhythms.


Aging is a major risk factor for chronic diseases, including obesity, diabetes, cancer, cardiovascular disease, and neurodegenerative disorders1. Improvements in healthcare have increased life expectancy worldwide, but as the aged population increases, frailty and morbidity have become a public health burden. Several biomarkers of healthspan are used in animal models2,3 and humans4, ranging from levels of metabolites (glucose, cholesterol, fatty acids), biological processes (inflammation, autophagy, senescence, blood pressure) to biological functions (behavior, cognition, cardiovascular performance, fitness, and frailty).


Dietary restriction widely improves lifespan and healthspan1, and some of its benefits could be mediated by the circadian system. Interactions between circadian clocks and aging-related pathways, such as the sirtuin6,7,8, insulin/IGF-19, and mTOR10 signaling pathways, support this hypothesis; yet, the mechanisms are not fully understood. Here, we review the literature from animal models and human trials assessing the effects of feeding paradigms and food quality on circadian rhythms, health, and lifespan. Finally, we discuss concepts of circadian medicine as an opportunity for tailoring antiaging interventions.


In the last two decades we have come to appreciate that, in addition to caloric content, timing of feeding and fasting periods between meals influence wellbeing45,46. Recent findings highlight the influence of dietary restriction (DR) on circadian clocks, yet the mechanisms linking circadian systems with metabolism, epigenetic signatures, and age-related diseases remain to be elucidated. Furthermore, the role of peripheral clocks in regulating metabolic homeostasis during aging is still unclear. Understanding the mechanistic links between circadian regulation during dietary interventions is an exciting avenue that may provide insights on how to achieve optimal health benefits throughout life.
